Community Care

Patiently providing practical support to a client in the following domains:
  • Social Support: Providing social support to a person in their home or in the community.
  • Community activities and outings: Accompanying a person to activities of their choice, including social or community related activities.
  • Personal Administration. Assisting with personal administration includes managing calendars, reminders, appointments, correspondence, and other general administrative tasks.
  • General Transport: Transporting clients in either my vehicle or the client's vehicle.
Useful for providing:
  • practical support toward achieving your personal goals and daily tasks.
  • Support through the challenges that form barriers to independence or community engagement.
Community Care can be a single appointment or a regular, ongoing arrangement that will:
  • Restore and maintain confidence when engaging with the community.
  • Assist with tasks that might be difficult when alone or overwhelmed.
  • Reduce isolation by spending time together.
  • Occur in the safety and familiarity of your own home or a mutually agreed community location.
  • Never cost more than the current published price for equivalent NDIS Items. Go to TYPICAL COSTS to find out the current prices.
